We have released a new version of our Medic Alert Card, with the gracious assistance of Ranae Beeker, RN, Helen Cave, and Dr. Joshura Benditt, a pulmonologist at the University of Washington.
Anyone with FSHD should carry this wallet card, especially if they have breathing issues.
In addition to providing space to enter your emergency contact and medical provider contact, there are checkboxes for information that an EMT or ER doctor would need, should you be incapacitated and not able to provide this information yourself. Importantly, the card provides a QR code to a web document with detailed Emergency Medical Considerations. An EMT or ER doctor can scan and read this vital information on their smartphone.
Everyone attending the FSHD Connect conference in person received one. To make your own card, you can download the PDF, print it on card stock, and glue or laminate the cards together.
